What you need to know about the hair growth cycle

Each hair strand is a single entity with a complex structure. It consists of the shaft, the root, and a supporting infrastructure. The root is located beneath the skin, and the shaft is the visible part of the hair. The root is firmly embedded in a hair follicle, combining with the surrounding skin and connective tissue. The follicle is linked to a gland that produces oils that keep the hair hydrated and firm.

The root expands at the bottom of each hair strand into a structure known as the bulb. The hair papilla lies within the bulb and supplies the nutrients delivered by the blood necessary for hair growth. New cells are continuously formed near the base close to the papilla. The cells undergo a hardening and bonding process as they move upwards along the strand until they become solidified. As hardened cells accumulate at the base of the strand, they push the hair up through the skin layers, leading to visible growth.

The Hair Growth Process

Hair growth happens in three distinct phases;

  • Anagen (growth) phase, during which hair follicles are actively growing. Cells within these follicles divide rapidly to elongate the hair shaft. 90% of terminal hairs are typically in this growth phase at any given time.
  • Catagen (transitional) phase, which signals the conclusion of the growth period for hair. In this phase, the hair follicle separates from the dermis, the soft layer of cells beneath the outer skin layer. This separation interrupts the root’s connection to its blood supply.
  • Telogen (resting) phase, in which hair growth stops and the root is pushed out through the skin layers causing hair to shed.

Which hormones are most influential in male hair growth and loss?

Testosterone is the primary androgen, which differentiates males from females within a very short period after a fetus’s conception. Since it is the primary male hormone, testosterone is responsible for traits that we associate with masculinity, such as virility, aggression, and sex drive. For a long time, it was believed that male-pattern baldness was itself a sign that a man was more virile since it was taken to be a sign of higher levels of testosterone. The assumption was simple: more testosterone leads to hair loss, but it’s also a sign of a more virile man.

However, it’s more complicated than that. Dihydrotestosterone (DHT) is a potent androgen derived from testosterone through the action of the enzyme 5-alpha-reductase. DHT plays an essential role in developing male characteristics during puberty as a boy transitions into a man. Later, it can also significantly impact the hair growth cycle, particularly on the scalp. DHT is critical in regulating hair growth on the scalp, binding to hair follicle receptors, and influencing their progress through the growth cycle. 

While DHT can stimulate hair growth in other areas, like the face and chest, it can have the opposite effect on the scalp, leading to hair thinning and loss. Scalp hair follicles are particularly susceptible to DHT. When DHT is abundant, it can bind to androgen receptors in the follicles and lead to a process called miniaturization. Miniaturization causes hair follicles to shrink,  gradually producing thinner and shorter hair strands. Over time, the follicles can become so small that they can no longer support healthy hair growth, leading to conditions like AGA. 

So, it’s now been established that there’s no direct relationship between the absolute level of testosterone in a man’s blood and AGA. Instead, the most likely cause of male-pattern baldness is an inherited sensitivity to DHT

The essential features that need to be understood can be summarised as follows:

  • DHT forms from testosterone. 
  • Although testosterone levels remain relatively constant in the bloodstream, the conversion rate to DHT can vary widely among individuals.  
  • People can also have different levels of sensitivity to DHT. 
  • These variations are predominantly due to genetic factors. 

Thus, men with certain genetic predispositions can have hair follicles that are particularly susceptible to the miniaturizing effects of DHT, leading to the characteristic thinning and receding hairline seen in male pattern baldness. 

Can hair loss coming from DHT be stopped and reversed?

It is important to note that the absolute level of testosterone is not the main trigger for baldness. Many men with high testosterone levels do not experience significant hair loss, while others with lower levels do. The crucial difference lies in the sensitivity of hair follicles to DHT. 

Researchers specifically developed some treatments, such as Propecia (finasteride) and Avodart (dutasteride), to target DHT’s actions. Doctors commonly prescribe both of these 5-alpha-reductase inhibitors (5-ARIs) to treat benign prostatic hyperplasia (BPH). Finasteride is generally effective in slowing down hair loss, and in one study over a year of daily administration, around 70% of the subjects showed no further hair loss. Dutasteride is a similar drug that is not approved by the FDA for treating AGA but can be prescribed off-label if a doctor feels that other treatments are unsuitable.
